CSC Exam Requirements: Photos, Valid IDs and CS Form 100
Most rejected Civil Service Exam applications do not fail on qualifications. They fail on paper. A photo with the wrong name tag. An ID that does not show a birth date, with no birth certificate behind it. An unsigned consent form. This guide goes through every CSC online application requirement, one by one, so your documents pass evaluation the first time for the March 14, 2027 CSE-PPT.
CS Form No. 100 (Revised 2023)
This is the official application form for the Career Service Examination. When you apply online through OCSEAS or eServe, you do not hunt for a blank copy. You answer the questions on screen and the system generates the form for you. Print the generated form on legal-size paper and bring it to your personal appearance. Check every entry against your ID before printing. The name, birth date and other details on this form, your profile, and your documents all need to match.
The photo rule, in detail
You need four identical passport-size photos, taken within the last three months. Each photo set must show you holding a name tag. On that tag, your handwritten signature goes over your printed full name. Read that again, because this is where applications die. The signature must be handwritten, by you, over your printed name. A name tag made on a computer is not accepted. An old photo from last year is not accepted. A photo without the hand-held tag is not accepted.
Studios near CSC offices do these photos every exam cycle and know the format. Tell the studio it is for the Civil Service Exam and ask them to follow the CSC name tag rule exactly. Check the prints before you pay. If the tag is wrong, have them redo it on the spot.
Valid ID: original plus photocopy
Bring one valid government ID, the original and a clear photocopy. Accepted IDs include the Philippine passport, LTO driver's license, PRC license, SSS or GSIS UMID, PhilSys national ID, and other government-issued IDs with your photo. The ID is the anchor document for your whole application. Your OCSEAS profile is checked against it, which is why the step-by-step in our online application walkthrough tells you to copy your details from the ID, not from memory.
PSA birth certificate: sometimes required
You need a PSA birth certificate, original and photocopy, when your ID does not show your date of birth or your middle name. If your ID shows both, you can skip it. If it does not, order the certificate early. PSA delivery takes time, and filing opened November 2, 2026 with slots moving fast. Waiting on a birth certificate while your testing center fills up is a painful way to miss the March exam and land in the August 8, 2027 cycle instead.
Certificate of Consent
The Certificate of Consent is downloadable inside OCSEAS during the application steps. Download it, print it, sign it, then upload the signed copy as part of your documents. Bring the signed original to your personal appearance. An unsigned upload is a common, silly reason for an evaluation to stall, so sign it before you scan it, not after.
Dual citizens
If you hold dual citizenship under RA 9225, bring your Bureau of Immigration Identification Certificate as well. It proves the Filipino citizenship the exam requires.
The ₱500 fee
The examination fee is ₱500 for both Professional and Subprofessional. You pay it in person at the CSC office during your personal appearance, not online. Keep the official receipt with your printed form. If anyone online offers to collect the fee for you, walk away. That is not how the CSC takes payment.
Pack your envelope
The night before your appearance, lay out one envelope with: your printed CS Form No. 100, your original valid ID and its photocopy, your four passport-size photos, your signed Certificate of Consent, your PSA birth certificate if your ID needs backup, and cash for the ₱500 fee. Checking this list at home beats discovering a missing photo in the office queue. The homepage overview of the CSC online application ties these requirements back to the full filing timeline, and if your slot is secured, the only thing left is showing up complete.
